The study included 127 systematic, scoping and realist reviews, comprising 1,259 unique primary studies, and mapped the evidence according to intervention type, healthcare setting, drug focus, outcomes and methodological quality. Medication review was the most frequently evaluated intervention, while evidence on focused deprescribing, clinical decision support systems, comprehensive geriatric assessment, educational and multifaceted interventions was more selective. Important evidence gaps were identified in primary care and emergency departments and in patient-centred, safety, implementation and economic outcomes. Methodological quality was also an important limitation, with 72.4% of included reviews rated as critically low confidence using AMSTAR-2.
